We want to make a great first impression! Before your first visit to Millcreek Animal Hospital, please review the following suggestions:

Please call ahead to schedule your first appointment (803-359-1933). Our friendly staff will be happy to help you set up a time that is convenient for you.
If your pet has records at another veterinarians office, please contact them and ask them to FAX your pet’s records to our office (803-359-6236).
Review and complete the forms below in advance of your visit. This way, you can sit, stay and RELAX with your pet when you come in for your first visit with us.
